Please complete your entry form correctly.  Make sure that you, the swimmer, sign it and return it to Mary Mocklow with your payment either by handing it in on a Sunday night at the desk or by giving it to your Coach or sending it to:

You cannot send this form directly to Worcester County as they will not accept it and you won’t swim.  Your form must be returned to the desk by the 7^th January 2007 or to Mary by the 14^th January 2007. 

If you are unsure as to how to complete the form or need any help, please ask your Coaches.  Remember, all times entered must be verified and achieved after 26^th April 2006 and should have been given to Stewart Nicklin to enter on the database.  Please ensure you have done this.
